Transitional Home Care
A closer look at the professionals behind our Transitional Home Care program.
Our licensed nurse practitioner will conduct your initial in-home, telephone and video visits. They’ll further identify your needs from both clinical and social perspectives, as well as coordinating care with your personal doctors to close care gaps before issues arise.
Working in support of a nurse practitioner, one of our registered nurses will make post-discharge phone calls and visits to identify and mitigate risks leading to hospital readmission or emergency room revisit.
