Accessing Your Personal Health Information 


As a patient of St Tammany Health System, it is your right to obtain copies of your medical records. Requesting your medical records, can be done via online request, fax, or mail.

Billing and financial services

 

To obtain copies of medical records, a patient or their representatives must complete a Release of Information form. The form must be signed and dated by the patient or patient’s representative, and it must include the name and address of the individual who is to receive the copies of the record, the information being requested, and the treatment dates.

 

After receipt, our team will review for all required elements and will process your request in accordance with federal and state laws governing your privacy and medical records. Allow 5-7 days for this process.
