Home Care Services
St. Tammany Health System's Home Health and Hospice programs provide rehabilitative, palliative and supportive care to patients confined to their homes and under their physician’s care. The professional medical personnel are committed to providing quality care while working with patients to meet their goals. They handle all Medicare, Medicaid, private insurance and workers' comp billing for our patients and accept assignment on covered service.
PUTTING THE 'CARE' IN HEALTHCARE.
For our Home Care Services teams, it’s all about one thing: compassion.
On the Northshore, family is central to everything we do. We take that seriously at St. Tammany Health System.
That’s why our Home Care Services programs have become such an integral and appreciated part of our community. Here, our patients aren’t just customers. They’re members of our extended family, and we treat them that way.
Providing rehabilitative and supportive care to patients confined to their homes and under their physician’s care, our professional and medically trained team members are committed to providing quality care – with an emphasis on the word “care.”
At St. Tammany Health System, our job doesn’t end at the hospital doors, thanks to our Home Care team, members of which still make house calls – and who prove St. Tammany Health System is much more than a hospital.
Primary Wellness Home Care
You know the saying: An ounce of prevention is worth a pound of cure. In that spirit, St. Tammany Health System has introduced its Primary Wellness Home Care program, which offers patients an annual checkup, taking place either in your doctor's office or in the comfort of your own home, by one of our highly trained providers to support and complement your regular doctor's care.
In addition to helping detect disease early, when treatment is more effective, annual wellness visits can also provide those who have chronic conditions, such as heart disease or diabetes, with tools and resources to help manage their condition.
The best part: It’s all covered by Medicare Part B, so it comes with no additional expense for qualifying patients.

Easy as 1, 2, 3
This free, “once in a lifetime” benefit is for patients who are within the first 12 months of enrolling in Medicare Part B. During this in-home visit, the provider will record your height, weight, blood pressure, body mass index and vision. Among other things, the provider will also go over your medical history and provide you with information and referrals specific to your personal healthcare needs.
Patients who have been enrolled in Medicare longer than a year will skip the initial preventative physical exam and instead receive their first annual in-home wellness visit, in which many of the same basic health assessments are performed. That includes, among other things, a review of your family and medical history, a screening to detect cognitive issues, general health advice, as well as referrals as needed.
Once you’ve had your initial visit – whether it’s the initial preventative physical exam or an initial annual wellness visit – you qualify for an annual in-home wellness visit. During this visit, your care provider will record your height, weight, blood pressure, body mass index and vision, in addition to reviewing your medical history, performing a health risk assessment, screening for cognitive issues, and providing health advice and referrals as needed.
