COVINGTON – St. Tammany Health System’s Home Health team has earned Home Health Heart Failure certification from the American Heart Association. This certification recognizes the health system’s alignment with AHA’s rigorous, science-based standards for heart failure patients, including program management; patient and caregiver education; and support, care coordination, clinical management and quality improvement.
“This certification affirms the lifesaving mission and goal of the American Heart Association to help every person everywhere have the opportunity for a longer, healthier life,” said Kyle G. Lavergne, DNP, APRN, FNP-BC, American Heart Association volunteer, founding member of the certification standards writing committee and vice president of clinical programs for CenterWell Home Health in Atlanta. “We’re pleased to have St. Tammany Health System join the national network of American Heart Association Home Health Heart Failure certified agencies.”
The certification signifies confidence that:
- An agency’s heart failure program is based on standards aligned with the American Heart Association’s science-based guidelines.
- The agency is recognized by the American Heart Association, based on professional evaluation criteria designed by experts in heart failure and home care.
- Care is coordinated from pre-admission through working with the home health agency to discharge.
Founded by members of the communities it serves, St. Tammany Health System has been caring for Northshore families since Dec. 1, 1954, with the opening of its flagship St. Tammany Parish Hospital. Since the 1980s, its Home Health Department has worked to bring skilled nursing, rehabilitation therapy and supportive care directly to patients in the comfort of their own homes.
Today, STHS Home Health is one of five divisions of the health system’s award-winning Home Care Services. The others are: Hospice, Palliative Care, Transitional Home Care and Primary Wellness Home Care. All are dedicated to serving Northshore families by delivering physician-directed care in the home setting while also reflecting the same compassion and commitment to quality that defines the broader health system.
